We provide two separate forms for your convenience (see below) – the first is for groceries and non-alcoholic beverages; the second is for alcohol.
Once the forms have been completely filled out, they may be faxed to us at 970-871-6372.
If you have special requests, they may be indicated in the blank spaces provided on the form, and we will do our best to accommodate them.
All orders must be received by 9 am MST at least 2 days prior to arrival, and sooner is much appreciated!
PLEASE NOTE: If you wish to purchase alcohol, you must send us a copy of your Driver’s License, either by e-mail or fax. (e-mail is often preferable due to variability of fax transmission quality, but either may be used.)
